Aristoi Classical Academy Athletics Participation Requirements
Effective August 1, 2022
- Athletic Participation Forms must be submitted annually before a student participates in any school tryout, practice, athletic class, open gym, athletic competition, or travels with an athletic team for any purpose.
- For the health and well-being of students, Aristoi Classical is requiring that all athletes in grades 5th-12th participating in organized school-sponsored sports have a new Pre-Participation Physical Examination (PPE), also known as a “sports physical”, given prior to the start of each school calendar year.
- A Pre-Participation Examination (PPE) differs from a “well-child” exam in that the focus is to “clear” the participant for a specific activity. A PPE may be scheduled at the same as a well-child visit. A PPE can help find health problems that could stop your child from competing safely.
- Parents should print a physical and medical history form to take to your physician or school-sponsored PPE event. The required form provided by UIL is available HERE.
- To streamline the process, parents are encouraged to fill out the Medical History portion of the form prior to arriving at the students’ appointment or school-sponsored PPE event.
- Students are required to use the Pre-Participation - Physical Examination Form provided. NO OTHER Physical Examination Form will be accepted.
- Parents and students are encouraged to watch the following Cardiovascular (Heart) Related Educational Videos prior to filling out the Medical History portion of the PPE and before arriving at the students’ PPE appointment.
